I-20 Request for Continuing Student

Continuing or returning F-1 visa students can use this form for any of the following purposes:

• Dependent I-20 Request
• Initial I-20 Request (returning to Lewis from authorized leave)
• This is only for continuing students who have been out of the United States for more than 5 months and wish to re-enter the U.S.
• If you are a new student who has not yet been admitted to Lewis and would like to enroll and obtain an I-20, please email international@lewisu.edu.
• Reinstatement I-20 Request
• I-20 with Travel Endorsement Request
• Updated I-20 Request (lost old copy, biographical, financial, or program updates)
